Sprout Organization is a non-profit dedicated to supporting individuals experiencing vulnerability in our community. Our Recuperative Care Program provides transitional care for individuals discharged from the hospital who need a safe environment to recover.
Position Overview
The RN provides minimum-service clinical oversight in accordance with CalAIM requirements. This is not a continuous or skilled nursing care role. The RN ensures the clinical safety of program participants through daily assessments, care coordination, and guidance to Community Health Workers (CHWs).
Duties and Responsibilities
- Conduct daily clinical walk-throughs and visual wellness assessments
- Review CHW notes from all shifts and provide follow-up instructions
- Review hospital discharge paperwork for all incoming participants
- Perform medication reconciliation (review only — no administration)
- Take vitals when clinically indicated and document findings
- Coordinate with primary care providers, clinics, and home health agencies as needed
- Determine need for hospital transfer when appropriate
- Maintain accurate and timely clinical documentation
- Develop and update limited clinical care plans
Minimum Qualifications
- Valid California Registered Nurse (RN) license in good standing
- Experience in care coordination, discharge planning, or community health preferred
- Ability to work independently and communicate effectively with a multidisciplinary team
- Strong documentation skills
- Commitment to serving diverse, underserved populations
Schedule
Part-time. Specific schedule to be determined based on program needs.
